Requirement to Continue FEHB Enrollment into Retirement • To be eligible to continue their FEHB enrollment into retirement, the employee must: • be entitled to an immediate retirement (one which commences within 30 days of separation), and • Have been enrolled during the 5 years immediately prior to retirement, or • Since first eligibility to enroll and during all periods of eligibility
FEHB Documents • All FEHB documents on file with the agency should be submitted with the retirement package • SF 2809s or other enrollment forms • SF 2810s • Evidence of coverage under spouse’s FEHB enrollment • Evidence of TRICARE/CHAMPUS enrollment
Documentation of 5 Years Coverage - FEHB • The retirement application package must include: • Proof that the retiring employee meets the eligibility requirement (documentation of 5 years coverage, including all changes in enrollment), and,
Documentation of 5 Years Coverage - FEHB (Cont) • A memorandum noting any circumstances that would help us determine the employee’s eligibility to continue coverage. • This memorandum can be used to detail the continuous coverage of an employee for the 5 years prior to retirement when documentation is otherwise unavailable.
Documentation of 5 Years Coverage - FEHB • Most FEHB enrollments are now done on-line • In lieu of SF 2809, a report(s) from the on-line system or screen print of the on-line enrollment which document 5 years of coverage may be submitted • To be acceptable, the report(s) or screen print(s) must show the old plan, the new plan and the effective date of each change in enrollment
Alternative Documentation of 5 Years Coverage - FEHB • If the on-line report(s) or screen print(s) do not show the old plan, • Contact the carrier to fax latest enrollment or verify enrollment date of the old plan and document the contact • A signed memorandum detailing the employee’s continuous coverage for at least 5 years may be submitted • Corroborating documentation showing payroll deductions for at least 5 years should also be submitted
