Dexact-f backgrund forskning Hepatocyte growth factor (HGF) • Vid akut inflammation och infektion produceras HGF med hög affinitet till komponenter av extracellular miljö (färgomslag) • Vid kroniska inflammationer produceras HGF som inte binder sig till extracellular rummet och är biologiskt inaktiv.   5. Fallbeskrivning: 48 årig kvinna med ulceros kolit som var inlagd pga blodiga diarre. Det växte klostridium deficile I fecesodling och pos toxin. Behandling med Metronidazol hade ingen effekt.   7. Figure .Ligand binding affintity and concentration of HGF in feces and serum. The binding affinity to dextran and monoclonal anti-HGF ab (MN) was analysed by surface plasmon resonance (SPR), in feces samples of patients with infectious gastroenteritis patients caused by Clostridium difficile, IBD and colon cancer (A and B), and in serum of patients with IBD (black column) (C), and healthy controls (white column). The feces concentration (ng/ml) of HGF was measured by ELISA (D). Funktion - Dexact-f • Testremsan är ett semikvantitativt påvisande av HGF i feces samt pH-värdet • pH • Normalt f-pH = 5-6 • Alkaliskt pH - tarminflammation • f-pH 9-11 kan tyda på allvarlig kolit, SIRS och sepsis eller organsvikt • Lågt f-pH – malabsorbtion eller vissa bakterieinfektioner • pH-värdet kan avspegla graden av inflammation i tarmen och förutsäga prognosen vid allvarliga tillstånd • Både positiva och negativa färgresultat har betydelse vid tolkningen av patientens symtom Hepatocytegrowthfactor (HGF) Vid skada i tarmen en tillväxtfaktor (HGF) som är viktig för läkningen Infektion skadar tarmen och Dexact-f detekterar alla former av infektioner i tarmen (virus, bakterier och parasiter) Kronisk inflammation – HGF binder ej in till stickan

  12. Dexact-f har validerats på 536 patienter, totalt 1500 inklusive version 1.0. Utfördes blint innan odling/PCR svaren blivit färdiga (i tabell –enbart verifierade test med konventionella metoder inkluderade) Validering Sensitivity: 92,3 % (de som rätt identifieras med inf.) Specificity: 93,5 % (de som korrekt identiferas ej ha inf.) Pos. Predicitive value: 93,6 % Neg. Predictive value: 92,1 % (hur man kan lita på neg res. ej inf. - ej isoleras i onödan) Accuracy: 92,9 %

Dexact-f clinical use in establishment of diagnosis prior to reference test methods • Case report 2 • 60 years old woman • Levaxin-subs hypothyroid • Heredidity for heart diseases • Back pain, deodonal ulcer since several years ago. • CT scan ok • Admitted 28 th March for diarrhea in 3 weeks, painfull tenesmus • Rectoscopy shows severe infalmmation and ulcers. • Dexact-f negative 2nd April • Feces cultures negative • Coloscopy and biopsy verifies ulcerous proctocolitis 16 June • Case report 1 • 75 years old man with microscopic colitis. • Whipple tumour operation 2009 • Recurrent diarrhea and stomach pain • Septicamia in May 2012 • Admitted for diarhhea, fever, shivering. • CT scan stomach and echocardiography ok • Dexakt-f high pos at admittance with very high pH resembling generalised inflammatory response, SIRS, Sepsis. • Neg F-Calprotectin, Neg FHb, Neg faeces cultures • Blood cultures yielded growth of E Coli, Klebsiella, Enterococcus faecalis and Enterobacter • Presence of 4 different bacteria in blood indicates penetration of bacterial flora from bowel to circulation in spite of normal CT scan. pos. Dexact-f, High f- pH – 4 bacterail stams in blood, - Susp SIRS neg. Dexact-f – Debute of chronic inflammatory bowel disease • Case report 4 • Healthy 43 years old man • Dirrhea 20 times/day, fever, pain in stomach since 2 days. • CRP 188, PKINR spontaneously 1.6 • Normal liver enzyme tests • Received one dose cefotaxim iv at emergency Department • Dexact-f pos pH 7.0-8.0 at admittnce 26th June • Antibiotic theray disrupted, iv rehydration. • CRP decreases spontanously and PKINR normalised • Faeces cultures 30th June reveals Campylobacter jejuni. • Case report 3 • 75 years old woma with poly neuropathia and parkinsonism • Admitted March 2012 for diarrhea in 2 weeks. • Dexact-f 28th March neg, low pH < 5.0 • Negative faeces cultures and high faeces Calprotectin. • Coloscopy 18th April shows diverikulosis • Biopsies reveal microscopic colitis June 2012 • Diarrhea continues neg. Dexact-f, low faeces pH – Debute chronic inflammatory bowel disease pos. Dexact-f, high faeces pH – bacterial infection

  16. Dexact-f och feces calprotectin tillsammans kunde forutsäga svåra tarmsjukdomar Fecesprov samlades may 2011 från patienter som sökte till vårdcentral pga tarmbesvär. Sedan följdes patienterna och i august 2012 registerades diagnoser. Som visat kombination av pos Calprotectin och negativ HGF kunde förutsäga en seriös tarm sjukdom I tidigt skede (14/19; IBD = 11, Colon cancer = 4 and Celiaki = 4).

  17. pH och mortalitet – resultat Dexact-f • Jämför med Ref: Prognosis impact of fecal pH in critically ill patients; Critical care. 2012 ”Once normal flora is altered, decreases the number of beneficial bacteria. This causes fecal pH to increase, which could then allow harmful bacteria to proliferate because of the alkaline environment. Harmful bacteria may then produce lactic and succinic acids, which lower the fecal pH and injure the intestinal epithelium. Finally, the injured intestine does not absorb lactose, which causes the fecal pH to be severely acidic.”

Fördelar/brister med Dexact-f • Snabbt test – svar direkt • Välkänd metodform – enkel att använda • Stabil i rumstemperatur • Minskar antibiotikaförskrivning • Diskriminerar mellan fall som behöver isoleras eller inte Torra feacesprov, mycket högt eller lågt pH samt mycket låg koncentration av HGF kan påverka resultatet. • Anledningen är att fecesprovhar många olika alternativ i konsistens, färg och innehåll. Mängden av HGF skiljer sig mycket i olika prov. Feces-pH skiljer sig mycket i olika prov. Det finns idag ingen annan test som kan svara på samma frågor som Dexact-f och därför finns ingen golden standard.
