2014 Open Enrollment Highlights • HealthPort continuously assesses benefits costs in order to determine any necessary program changes while striving to provide a comprehensive benefits package to all eligible associates. Here are the 2014 highlights: • We are excited to be offering a dual option High Deductible Health Plan with a Health Savings Account along side our standard PPO plan • The PPO plan will still be offered however, there are some benefit changes. • The Standard will continue to provide the dental and vision insurance with no changes to benefits or price. • We will be changing Health and Dependent Care Spending Account administrators to Benefit Resources Inc. • The critical illness, accident, and legal plans have been discontinued.
Considerations for 2014 Open Enrollment • 2014 Open Enrollment is taking place October 14th – 25th • Choose medical plan carefully - elections take effect January 1, 2014. Pre-Tax elections cannot be changed during the year unless you have an IRS qualified event • Our new FSA vendor is Benefit Resources Inc.(BRI) • You must re-enroll in the Health Care and Dependent Care Flexible Spending Accounts (FSAs) every year
Enrollment Instructions • We are having an active enrollment. You have to make an election to continue coverage. If you take no action, we will assume you are waiving coverage. • Go to the UltiPro Homepage and select Myself > Open Enrollment • Select the current open enrollment session • Verify and add any new dependents • Follow the steps to enroll or decline in each benefit
UHC Choice Plus Plan • Predictable copayments • Coverage for non-network care available at lower benefit level • See any network doctor, including specialists, without referrals • No need to choose PCP • 100 percent preventive care coverage in network • Care management if you require a hospital stay • Prior-authorization may be needed for certain services
UHC High Deductible Health Plan with Health Savings Account • Combines a tax preferred Health Savings Account (HSA) with corresponding High Deductible Health Plan (HDHP). • HealthPort will match dollar for dollar up to the first $500 for single coverage or the first $1000 for family coverage. • Preventive services are covered at 100%. • Set aside pre-tax dollars to pay for current medical expenses or to save for future medical expenses. • You must not be covered by any other health plan, including a spouse’s plan, a flexible spending account, or Medicare. • 2014 HSA Maximum annual contributions (as set by the government) are $3,300 single / $6,550 Family (including company matching contribution). Catch-up contributions (age 55 or older) are limited to $1,000.
What is A Health Savings Account (HSA) • A personal bank account that you own. • You can use the money to pay for qualified medical expenses today • Or, you can save it for a future medical need — even into retirement. • You won’t have to pay federal income tax on: • Deposits you or others make to your HSA • Money you spend from your HSA on qualified expenses • Interest earned on the HSA Proprietary Information of UnitedHealth Group. The Standard: Supplemental Life Insurance Supplemental Employee Life Supplemental Dependent Life Evidence of Insurability (EOI) will be required for Employee, spouse and dependents if you have not previously had coverage.
The Standard: Short Term Disability Evidence of Insurability (EOI) will be required during Open Enrollment if you don’t have current coverage. * Effective 1/1/14, benefit deductions will be taken out of disability checks
