  2. Q1. Should students be permitted to fill out the evaluation repeatedly, with only the last one counting, or should they be restricted to only one attempt? • A. The Senate Technology Committee recommended Repeatedly.

  3. Q2. Are the data aggregate across all courses, or just on a per-course basis? And should it be public? • A. Aggregate data across all courses, not individually and make the data public.

  4. Q3 Should the data from the pilot be left in the system (hence, usable for RTP) or should they be erased before the application goes into production? • A. Since the pilot system met the Senate’s requirements, I&IT recommends that evaluations from Winter, 2007 pilot shall be retained and permitted to be used for RTP purposes.

  5. Q4. Does an evaluation form originate from the department of the instructor or the department of the course, when they differ? • A. Department of the course.

  6. Q5. Shall a department chair designate the person entrusted with inputting the evaluation form for a department? • A. I& IT is willing to do data input as long as the forms are official.

  7. Q6. Should use of online evaluations for non-online be encouraged, allowed, or prohibited? • A. Instructors using on –line evaluations shall not be permitted to use paper evaluations, since there is no way to simultaneously respect confidentiality and assure that a student will not have evaluated the course twice.

  8. Q7. When is the earliest date at which the results of evaluations should be available to instructors? • A. For those departments that have submitted evaluation forms by the beginning of the 5th week of the quarter

  9. Recommendation • Provided a department’s forms are on-line, all instructors teaching courses in that department should be permitted to use online evaluations.
