Moffat project aims To create effective partnership working between the NHS and Social Work Staff, Carers Centres and Carers To promote early identification, assessment, intervention and supports to carers To ensure carers are supported as partners in care To support carers health and wellbeing

Pilot sites Location: Stobhill Hospital Drumchapel Hospital Mansionhouse Unit Victoria Infirmary Southern General Leverndale Hospital Paisley Royal Alexandra Hospital Glasgow Royal Infirmary Lightburn Hospital Development Area: Medicine for the elderly Emergency receiving unit Physical Disability Rehabilitation Unit Palliative Care (services for people with life threatening conditions other than cancer) Mental Health in-patients

  6. The project in practice Dedicated workers to work in partnership with Health and Social Care Staff Delivering Carers Awareness Training Providing information and support to carers Working to enhance the discharge process Financial savings

  7. Challenges The pilot projects need to be sustainable! We need to recognise the demands on NHS and Social Care staff…something else to do? Policy and legislative framework, we have a duty to identify , support and inform carers!
