What we will cover. What messaging is all about? What do CRs need to be doing? How is this being taken forward? What about NPfIT?. Its all about Interoperability. MessagingXMLInteroperability

Cancer Registration, England
3. Its all about Interoperability Messaging
XML
Interoperability
The ability of two or more systems or components to exchange information and to use the information that has been exchanged IEEE, 1990
4. So why the cancer registries?
5. Action Programme (DH 2001)

So we need to exchange
And we need to useSo we need to exchange
And we need to use
7. Cancer Registry Needs
Data currently collected by different mechanisms
Standard electronic data
NHS Trusts to prove and adopt
To use XML
To roll out use of the Cancer Data Set This slide to outline briefly the current position with the Cancer Registry data collection and where they are looking to go. Mainly manual data collection by various means.
Introduce the vision for consistent electronic data collection to ensure good data quality and improve data collection coverage and timeliness
The government requirement is to use xml as the standard inter system data exchange (message) format
Any cancer registry message developed will be built using the National Cancer Data Set items. This slide to outline briefly the current position with the Cancer Registry data collection and where they are looking to go. Mainly manual data collection by various means.

8. So how is this happening? Modernisation Funding
Network Liaison Roles
Cancer Dataset
Message-based Systems
Rules-based Interpretative Systems
9. Cancer Dataset Cancer dataset is a Fundamental Standard
Operational instances of audit dataset messages based on fundamental standard
Cancer waiting times
LUCADA Lung
Head and Neck
Is this sustainable? This slide to give some back ground to the development of the cancer data set to strategic standard and the various operational develops that have been started/approved based on this.
Cancer data set approved by Information Standards Boards as a strategic standard in January 2003. Work continues to develop and pilot remaining cancer areas (Haematology, Childrens, Palliative and Primary Care)
Cancer dataset consistent with common items in National Cancer Registry data model.
Waiting Times Data Collection from Dec 2002
Potential to act as trigger for Cancer Registration
NCASP National Clinical Audit Support Programme
Breast, Lung, Head & Neck and colorectal 2003
XML message has been developed for LUCADA

Define the Key Event Requirements
Define the Metadata
NHSIA
Build the XML Schema for Cancer
Build messages using the Cancer Dataset
Formal routes through ISB
Testing, Pilot, Standard, Adoption
13. So what messages are there?
14. What does the XML schema look like?
15. Current registration process
16. Change in data sources over time
17. Implementation Stages Stage 1
test the transmission of Cancer Registration data items from NHS Trusts to Cancer Registries using NHS XML schema
data items collected in current mixed formats and converted into messages capable of transmission to Cancer Registries
18. Stage 1 process for registration
19. Implementation Stages Stage 2 will utilise the new services to be available from the Secondary Uses Service (SUS) of the NHS Care Records Service (NCRS). This service will use the Transaction Message Service (TMS) feeding the National Spine via XML schema produced from an HL7 Version 3 Reference Information Model (RIM). The Cancer Registries will then take the data feeds as a user of the NCRS/SUS
20. Final process for registration

23. How does this fit with the world? Why bother NPfIT is coming
How well does our model fit HL7?
I bet someone stops you
.
Why didnt you use UML?
Cancer as an exemplar?
24. Looking forward - NPfIT Electronic Booking
