2. Structures • Survey teachers to determine Academic Intervention Needs. • Arrange for a place to work with students. • Determine a time to work with students. • Decide the information needed from teachers. • Notify teachers of time, place, requirements.

3. Strategies • Advertise your Success lab and Contact your teachers weekly. • Establish working guidelines within the Success Lab. • Be completely available to students during the Success Lab. • Have Materials available. • Check student work and congratulate them for working.

4. Data • Keep records of students served. • Follow up with teachers and celebrate your successes! • Students served in December: 110 • Out of 31 8th Grade students who were failing Math, 13 passed after working in the Success Lab.

5. Chestnut Log Middle School Graduation Coach Ann Otwell 770-651-5193 November, 2007 Teachers, In an attempt to provide additional intervention for students who are not completing assignments, I will begin a SUCCESS LAB next week on Tuesday and Wednesday. STUDENTS CAN BE REFERRED TO THE SUCCESS LAB FOR AN ADDITIONAL OPPORTUNITY TO COMPLETE REQUIRED WORK. If you wish to refer a student to come to the Success Lab during a Connections Class, please clear it with the Connections Teacher first. Attached is a referral form which will help us use time more efficiently. Please use this form to refer each student. You may put the referral in my box, email it to me (completely filled out), or hand it to me personally. I am asking that the forms be turned in the day before the students are to report so that I will be better prepared to work with them and will have the opportunity to clarify any questions I may have about assignments.
