Improving The Multi Disciplinary Care and Management of Patients at Risk of Developing Venous Thrombo Embolism (VTE) in Rotherham Hospital NIHR CLAHRCfor South Yorkshire National Institute for Health Research BACKGROUND TO THE PROJECT In 2005 the House of Commons Health Care Committee identified that each year 25,000 people die from hospital Acquired VTE VTE is the most common preventable cause Of hospital death Chief Medical Officer made recommendations to improve current practice - in 2010 the DoH and NICE Published Guidance prioritising the prevention of VTE across the NHS. Equivalent to one jumbo jet crashing every week 25 times more people die from VTE than MRSA The cost to the NHS to manage and treat patients diagnosed with hospital acquired VTE is estimated at over £640 million a year METHODOLOGY FOR THE EVALUATION The project is evaluating different aspects of VTE management and care using various methods of staff and patient facilitation and engagement based upon evidence based practice. In collaboration with CLAHRC South Yorkshire The Rotherham Foundation Trust is undertaking this project,designed to facilitate the multidisciplinary (MDT) in improving the care and management of patients at risk of developing VTE from admission to discharge through the implementation of NICE guidelines and quality standards Evaluation of the role of Quality Improvement Teams (QIT) in knowledge transfer to the MDT Evaluating patients experience of written and verbal information; how this is interpreted and used by them, and engaging them in the development of patient information booklets Staff engagement strategies to be used throughout the project Identifying training and education needs of Quality Improvement Team members and the MDT using pre project implementation questionnaires to examine; knowledge and attitudes to VTE risk assessment, management and care Identifying the role and context of Quality Improvement teams using Interviews and field notes Facilitation of the Quality Improvement team members in quality Improvement, change management, and service Improvement, methods Providing evidence based Knowledge of VTE and management to Quality Improvement Teams and the MDT On Going Support of Quality improvement teams and ward staff implementing change, and education of staff managing patients at risk of Venous Thrombo Embolism Evaluation of Quality Improvement Teams and translation of knowledge to the MDT through facilitation using post implementation questionnaires, interviews and field notes VISUAL Posters Leaflets Screen savers Newsletters EDUCATIONAL Teaching materials Meetings Study sessions/days MANAGEMENT STRATEGIES Rotherham Hospital VTE Steering Group integrated with this VTE Project Group to ensure development of VTE management and prevention is appropriate for Rotherham Hospital This project forms part of the NIHR CLAHRC for South Yorkshire: to find out more please contact kate.bray@sth.nhs.uk/ marie.lowe@rothgen.nhs.uk/ www.clahrc-sy.nihr.ac.uk
