  2. Here’s what we know Suicide is the 3rd leading cause of death in 15-19 year oldsand the 4th leading cause of death in 10-14 year olds. (SAMHSA 2011) • Risk factorsmay include: • History of previous suicide attempts • Family history of suicide • History of depression or other mental illness • Social Isolation/Bullying • Alcohol or drug abuse • Stressful life event or loss • Easy access to lethal methods • Exposure to the suicidal behavior of others • School Exclusion Introduction

  3. IF ANY STAFF LEARNS OF: A suicide attempt, gestures, and/or share thoughts/talk of suicide including those thoughts expressed in writing, art or other forms • IMMEDIATELY REPORT TO COUNSELOR OR SCHOOL PSYCHOLOGIST ANDADMINISTRATOR • Implement the suicide protocol When do we use the protocol?

  4. SUPERVISE THE STUDENT! Under no circumstances should the student be allowed to leave school or be alone until a screening has been completed and a Student Support/Safety Plan has been generated What about the student?

  5. If the student is in imminent danger- call 911 and their parent/guardian Imminent Danger

  6. If the student is not in imminent danger…begin the Level 1 screening process. • The school counselor, school psychologist, or administrator will interview the student and complete the screening form. • Then consult with qualified school staff (District Office, School Psychologist, Counselor, Administrator, School Nurse) • Contact parent/guardian • Consult with qualified mental health professional as necessary (e.g. Multnomah County Crisis Line- 503.988.4848, provider in School Based Health Clinic, or student’s current therapist) After the student is interviewed using the suicide screening form- the team will determine next steps. If the team determines that this is an unfounded immediate concern-document that on the screening form and complete a student support plan as needed. If the team determines that a more extensive screening needs to occur- move to Level 2 Level 1 Screening

  7. If an immediate Level 2 screening is necessary, call the Crisis Line 503-988-4888. Alternatively, the parent/guardian may transport their child to the hospital or mental health provider. • If the Level 2 screening does not need to be immediate-complete PPS mental health service request form. • The student will then be referred to a qualified mental health professional • School-Based Mental Health Counselor • Multnomah County Mental Health Consultant • Call Center: 503-988-4888 for provider referral • Other community based provider • IF UNABLE TO OBTAIN PARENT/GUARDIAN CONSENT FOR AN IMMEDIATE LEVEL 2 SCREENING - CALL 911 to bypass the need for consent. Level 2 Screening

Prior to the student’s re-entry to school post a mental health emergency, obtain a release of information to access the student’s current mental health recommendations. • Have a re-entry meeting that includes a student safety plan. The safety plan must include input from the student, the family, and the current mental health provider. • Direct student and family involvement can help the student to regain some sense of control upon re-entry.
