Summary Update November 2013 of emerging themes, progress and achievements. Aim: Working with Home Care Providers on future service visions and workforce development plans to support, people living with Dementia at home Emerging Themes: • Identify resources / education and examples of good practice. • Links between Home Care providers and Social Care/Voluntary sector. • Different ways of providing knowledge and skills to staff. • The role of home care providers in avoiding admissions. • Information flows with more detail of the needs of Dementia clients. • Need for a more coordinated approach across organisations pre-admission to discharge. Target of 20 to 25 providers achieved Nov 13 (JB 10/12/13 )
Summary Update November 2013 of emerging themes, progress to date and achievements. New work stream Aim: To develop a framework/protocol to provide continuity of care to meet the needs of people with dementia and their families from pre-admission through to discharge to homecare providers. Objectives: • To work with 25% (20 to 25) of East Sussex homecare provider services, ESHT Dementia / Older peoples leads, and NHS /Social Care Dementia Champions. • To develop a partnership pre-admission to discharge framework/protocol. • To raise awareness with clinical staff and home carers of the specialist care needed by people with dementia and their families. • To develop a monitoring process including a case study approach. Development of a Joint Home care Good Practice Guide Work in progress: Working with the ESCC Homecare Providers Dementia Champions Forum. Collecting information and good practice examples from homecare provider and the wider Health and Social Care sector. Joint working with other projects and initiatives • ESCC and NHS Care Home initiatives and other projects • Health and Social Care Dementia forums. • The Pan Sussex EoLC Dementia Care Pathway. • ESHT Dementia Steering group, Carer Liaison /Facilitator and Dementia Care programme lead.
