Learning Objective Understand a framework for system and data integration to prevent and end homelessness, which includes three core components: • Cross-sector change leadership • Data integration • Data maturity (for data based decision-making) Understand how to apply this framework through critical lessons learned from communities
Cross-Sector System and Data Integration • Goal: • Maximize system investments, improve health, prevent recidivism, end homelessness. Objectives: • Identify and target the most vulnerable households • Effective and efficient cross-system interventions • Improved policies • Improved care coordination

Care Coordination Use Case Questions • How do we identify this group for prevention services and enrollment? • How can we facilitate joint-case planning and monitoring? • How can we create stronger cross-sector referral networks and identify eligibility? • How can we prioritize and target the most vulnerable persons accessing services?
System and Data Integration Framework—Critical Lessons: Data Integration • Data Integration can support eligibility processes, care coordination, and system planning • Identified leader(s) responsible for a) facilitating and completing data sharing agreements and b) overall data stewardship is critical to success
