  6. INTEGRATION PROCESS BENEFITS OF INTEGRATION • Trusting relationships allow you to drop your defenses and focus directly on the client • Sharing of resources and expertise leads to better client outcomes and improved service efficiency

ALZHEIMER SOCIETY OF YORK REGION • First ADP for persons with dementia in York Region • Support to caregivers tied directly to ADP integral to successful ADP participation and program utilization • Robust social work team • Caregiver Support and Education Program key to integration process and service delivery model

  8. BENEFITS FOR CLIENTS • Continuity for clients • Service navigation with on going case management, support and education • Standardized admission process and assessment process to ADP • On going involvement during clients participation in ADP

  9. BENEFITS FOR CLIENTS • Follow up after ADP discharge • Referral and linking to other community supports • Continuity in service delivery and philosophy (Person-Centred Care) • Resource to partners to ensure quality and best practice approaches

CURRENT DELIVERABLES Support and Education Program (AS York) • 1,500 families served by social work team • 250 of these families utilize the partner ADPs (200 Region of York + 50 York Central Hospital) • 15, 000 ADP partner ADP spaces filled • 36 client team rounds attended • 20 support group sessions facilitated

  13. CHALLENGES • Competition for $ with acute care sector • Need for a comprehensive plan for ADP services across York Region • Legislated centralized intake for all ADP • Implementation common assessment tool (InterRai CHA)
