  2. Heart Failure Clinic PURPOSE: to provide to patients with class III and IV Heart Failure and their significant others intense: • Medical follow-up • Education regarding: • Disease process • Medication therapy • Dietary and fluid guidelines • Medication purposes, use and side effects

  3. Heart Failure Clinic • Co-morbidities: • Diabetes • Hypertension • Hyperlipidemia • Atherosclerosis • Renal disease • Vascular disease

  4. Heart Failure Clinic • Lifestyle guidelines: • Living with heart failure • Proper exercise • Stress management • Daily weight • Proper nutritional needs and eating tips

  5. Heart Failure Clinic • Provide a consistent and easy to reach contact for problems • Conduit to additional services/referrals: • Heart transplantation evaluation • LVAD • Research programs (ie CorCap) • Palliative Care • Surgery • Home care for Inotrope and or Intervenous diuretic therapy.
