Indication:
Stable kidney function
Older patients
General screening
Creatinine Clearance = (140 – age) X weight (kg)
(Cockcroft-Gault) Scr X 72
(multiply by 0.85 for women)
Ideal Body Weight in kg (male) = 50 + 2.3 (Height in inches > 60)
(female) = 45 + 2.3 (Height in inches > 60)

Stable function
Middle aged patients
Black patients
Diabetic nephropathy
Status-post kidney transplant
More accurate than Cockcroft-Gault Formula under the above conditions
Underestimates CrCl in the absence of kidney disease
CrCl (ml/min/1.73m squared) = 186 X (SCr in mg/dl raised to the power of -1.154) X (age raised to the power of -0.203)
For females: multiply CrCl by 0.742
For Blacks: multiply CrCl by 1.210
