Payment of Subs 2014/2015

1. Payment of Subs 2014/2015 Name: ............................... Squad Number: .............. Annual subs for the 2014/2015 season are £65 per player. Annual subs must be paid in full before the 16th August (first friendly game). Players not meeting this deadline will be ineligible for selection by Rochdale Hockey Club until all monies have been paid in full. Training will take place at Oulder Hill High School, Rochdale on Tuesday evenings between 7-9pm. Training subs are £5 per month and can be paid as follows: Monthly payments of £5 per month. One off payment of £60 by 20th September (first league game) Training subs are compulsory for all members of the club whether training is attended or not. Failure to pay training subs will make you ineligible to play league games until monies are paid up to date. Repeated failure to pay will result in an appearance before the Club Disciplinary Committee and may result in a ban, fine or your membership of the club may be terminated. Training Subs should be paid either in cash or via standing order. Please have the standing order set up to transfer as close to the beginning of each month to make the administration as easy as possible.
