Transfer of Care - Respiratory Patient Identification • Exclusion Criteria: • No patient consent. • Patient is house-bound or in a Nursing Home. • Palliative patients. • . COLLATE reason for patient declining service • Inclusion Criteria: • Reside in Cambridgeshire / Peterborough • Asthma, COPD & Bronchiectasis pts. • On 2 or more medicines. • With Community Pharmacist > 3 months Does the patient consent to the service and for their information to be sent to their Community Pharmacist? Explain the benefits of the service to the patient giving them a Pt Information Leaflet. Yes No • Patient Referral • The Pre-Reg Pharmacist completes the Community Pharmacy Referral Form. • Calls the relevant Community Pharmacy to confirm agreement. • Fax the referral form to the pharmacist along with the top tips document following the approved process. Complete an audit trail to track transfer of information. • NB: Pre-reg Pharmacist to inform Community Pharmacist if delayed discharge. • FEEDBACK to Pre-Reg Pharmacist via the Referral form if not able to contact patient. Within 7 days , Community Pharmacist to contact patient via their preferred method. Have they been able to make an appointment with the patient? Yes No Has the patient arrived for their appointment with the Community Pharmacist? Repeat Call Yes No Repeat Call • Completion of the tMUR / NMS • Complete the tMUR / NMS and ensure all relevant information highlighted on the top tip sheet has been captured on the feedback form. • NB: Any queries with respiratory medicines prescribed, please contact Respiratory Specialist Pharmacist Jonny Davies on 01733 673 333, Pager 1990 • Feedback outcome • Community Pharmacist to fax a copy of the completed form to 01733 678 525 FAO: Courtenay Pearson • A copy of the form forwarded to the GP where necessary. • NB: The Community Pharmacist to inform the Pre-reg Pharmacist of any known changes to patients medicines post tMUR / NMS • REPORT the outcome of the service and make recommendations going forward. • Data Collection • Pre-reg Pharmacist to collate the data captured on the tMUR forms & reasons for no consent.
