PCWG Health Concerns • Problem oriented approach of healthcare • Important construct for Care Plan, C-CDA, etc • Generated huge amount of listserv debates on FHIR, Structured Doc, PCWG listserve • Debates still continuing • Project goal: • Develop DAM to inform development of • implementable clinical models • C-CDA template • FHIR resources

  3. Health Concern – what’s new? • Health Concern DAM work in progress • Conceptual model: draft available on PCWG wiki • Continue to evolve • Aim to be finalised at Phoenix WGM

  6. PCWG FHIR Resources • Development work to start during and post Phoenix meeting • Collaborations and inputs from: • PCWG published care provision materials • International programs outputs: NHS, Australia • SDWG, IHE … etc • Aim to have stable draft for FHIR QA by September WGM
