Paying With Your Flex Debit Card • The Flex Debit Card is a MasterCard that you can use to pay for eligible medical and dependent care expenses where MasterCard is accepted such as: • Physicians offices • Pharmacies • Daycare centers • Use you Flex Debit Card for the following: • RX co-pays • Vision co-pays and out-of-pocket amounts • Dental deductibles and co-pays • Orthodontic monthly payments • Post billed medical deductibles and co-pays • Dependent daycare expenses
Flex Debit Card Benefits • Instant access to Medical and Dependent Care account funds • Eliminates manually filing claims and waiting for reimbursement • When using the card there is no PIN number. When given the option to choose credit or debit you should choose credit • Extra cards are available at no extra charge for your spouse or dependent children (away at college)
Flex Debit Card When do I need to send receipts to EBS? • All receipts must be kept per IRS regulation. You will be required to submit documentation for some expenses. • All dental and vision expenses will require verification with the itemized bill per IRS regulations • Medical expenses, other than co-pay amounts will also require receipt verification • Notification will be sent to you via email or mail when more information is required. (If you cannot find your receipt, call your provider and have them fax the receipt to EBS using the 888 number on the back of your debit card) • If EBS does not receive the requested information, your debit card may be deactivated.
Fastest Acceptable Receipt • When you are at the provider (ie: vision provider or dentist), ask them to fax the receipt to the 888 number on the back of your debit card
Acceptable Receipt • An Acceptable Receipt includes: • Name of provider • Date of service • Explanation of services provided • Amount of charge • Name of patient
Explanation of Benefits (EOB) • Best receipt to submit is a copy of your EOB
Unacceptable Receipt • A credit card receipt is not acceptable. It does not show an explanation of services; therefore, EBS does not know what you are being reimbursed for.
Checking Flex Balances • Check your Personal Account Balance anytime • You have 24 hour access to your Personal Account • Log on to www.ebs-tpa.com and register to obtain access to your benefit information • Click on “Flex Debit Card Login” under the Quick Links
