04-01 Fire Equipment DamageInjury to Members • These are the steps that shall be taken when something happens to Fire Company members, or equipment. • The member who finds or damages equipment is to:
04-01 Fire Equipment DamageInjury to Members • Take damaged fire equipment out of service immediately. • Notify the Chief or in his/her absence, the Officer in Charge (OIC). • Place a note on the equipment out of service informational board in the radio room. • Fill out a Fire Company incident report.
04-01 Fire Equipment DamageInjury to Members • The incident report should include, but not be limited to: • Date • Time • Where it happened or where it was found • How it happened.
04-01 Fire Equipment DamageInjury to Members • If the Chief was not present, it’s the responsibility of the OIC to turn over the incident report to the Chief. • If a member is injured and he/she requires medical attention, then seek the medical attention needed first.
04-01 Fire Equipment DamageInjury to Members • The member shall notify the Chief or in his/her absence the OIC and then fill out the incident report at his/her earliest convenience. • Even if you’re not sure you are hurt, let the Chief or OIC know and fill out an incident report.
04-01 Fire Equipment DamageInjury to Members • The Chief or his/her designee shall then fill out the proper NYS Workman’s Compensation forms and submit a copy to the Town of Brunswick town clerk. • The Chief shall complete the NYS DOL form SH-900.2 and at the end of the year, complete the NYS DOL form SH-900 & NYS DOL form SH-900.1.
04-01 Fire Equipment DamageInjury to Members • Each of these forms shall be kept on file as per 12NYCRR part 801 entitled Recording and Reporting Public Employees Occupational Injuries and Illnesses.
04-01 Fire Equipment DamageInjury to Members • If a member is killed in the line of duty and the Chief is not there, the Chief is to be notified immediately. • If the Chief is killed, then the next Senior Officer shall take over and complete the investigation to find out the cause of death.
04-01 Fire Equipment DamageInjury to Members • In either case, all forms (NYS Workman’s Compensation and NYS DOL forms) shall be completed and kept on file. • Notification to PESH shall be made within eight (8) hours of a firefighter death on a scene or two (2) or more firefighters sent to the hospital from the same incident. • Their number is 518-457-5508.

04-01 Fire Equipment DamageInjury to Members • A message shall be left if the incident occurs after hours. • If there is a death, a Law Enforcement Agency shall be notified. • It’s the responsibility of the Chief or in his/her absence, the OIC to make sure if there were any eyewitnesses they also fill out an incident report documenting what they saw.
