Smoking attributable mortality Av annual deaths due to smoking related causes (aged 35 & over) expressed as a no and crude rate per 100 000 population. 1995-2001 Created using a formula (Callum 1998) that links smoking satus with attributable risks for specific causes of death known to be linked to smoking. Rating of good health/ wellbeing Self assessment of each persons general health over the last year. No & % classifying their own health as “not good”. 2001 census

  15. Mental health Prevalence - QMAS - Prevalence rate of patients on practice register of people with severe long-term mental health problems who require and have agreed to regular follow-up. Diabetes Prevalence – QMAS Number of patients who have a GP diagnosis of diabetes as a proportion of total list size DMF Teeth Mean number of decayed, missing or filled teeth in 5 year old children
