  4. Stakeholders from throughout the county • Visioning • We commit to having a vibrant, engaged, and compassionate community in which all individuals achieve their ideal physical, mental, spiritual, social, and economic well-being • Data • Prioritization • Root Cause Stakeholder Meetings

  5. 18.8% of adults report having difficulty participating in social activities because of mental or emotional health issues Data: Adult Mental Health

  8. Youth: • Only 35.7% of youth feel that their community cares about them quite a bit or very much • Adults: • 18.8% of adults report having difficulty participating in social activities because of mental or emotional health issues Data: Lack of Social Connectedness

  9. Top 3 Needs Identified • Mental Health (Adult Well-Being, Youth Suicide) • Youth Substance Use (Alcohol and Tobacco) • Lack of Social Connectedness • Dug deeper into “why” Prioritization and Root Cause

  10. Get more community member feedback • Two-Page package • Collected information for over a month • ECFE parent groups, support groups, LAC, faith groups, veteran groups, Allina Health staff, PTAs, ACT, TRIAD, Lakes and Pines…. Conversation in a Box

  11. Allina Health and ICPH are working to create strategies • Creating goals, objectives and action steps • Identifying community partners • Living document that will continue to be revised over the next 3 years Currently

GOAL: Reduce substance use/abuse to protect the health, safety, and quality of life for the youth in Isanti County • OBJECTIVES • Educate and communicate with communities around available youth substance use programs, resources, and services • Implement prevention and intervention programming and policies to address youth substance use Youth Substance UseAlcohol and Tobacco

GOAL: Improve mental health and well-being, and reduce suicide in the community through increased awareness and access to care, programs, and services • Objectives: • Educate and communicate with communities around available mental health programs, resources, and services • Promote skill building programming to help identify and assist those in crisis and increase resilience Mental Health: Adult Well-Being and Youth Suicide

GOAL: Support the community in promoting and enhancing meaningful social relationships and work to increase social connectedness across all stages and ages of life • OBJECTIVES: • Create a plan to support existing activities that build social connection • Support and create new activities that promote a sense of community spirit and belonging • Support local efforts to engage more people in community volunteering and citizenship activities Lack of Social Connection
