  2. Partnership for Patients (PfP) The Partnership for Patients has set an ambitious goal for all U.S. hospitals: reduce preventable all-cause harm rates 40% and readmissions 20% by end of 2013 in your organization.

  3. Partnership for Patients: A Movement Our hospital is part of a national movement for the triple aim to Put Patients First and make patient care: safer more reliable less costly

  4. Aim: To Reduce All Cause Harm All preventable harm must be addressed to reach the goal but these are the areas of focus to begin: (Hospital can star their priorities based on your hospital data) Adverse Drug Events (ADE) (includes Medication Reconciliation) Catheter-Associated Urinary Tract Infections (CAUTI) Central Line Associated Blood Stream Infections (CLABSI) Injuries from Falls and Immobility Obstetrical Adverse Events (First: Early Elective Deliveries) Pressure Ulcers Surgical Site Infections Venous Thromboembolism (VTE) Ventilator-Associated Pneumonia (VAP) Reduce Preventable Readmissions

Safe Care Not Only Important to Our Patients…. but also to Hospital Success Payment for care is now based on quality not volume • Value Based Purchasing • Medicare payment incentives/penalties to promote: • Achievement and improvement of high quality care • Annual Market update increased or reduced up to 2% by 2017 • Payment based on quality measures and Patient satisfaction (HCAPHS) • Higher scores = higher payments, low scores = lower payments • Nonpayment for Hospital Acquired Conditions • Higher payment withheld if condition not present on admission • Payment penalties for excessive readmissions

  6. The Hospital's Reputation is Transparent Public Reporting of Quality and Patient Satisfaction Scores Patients/families using Social Media to discuss care

  7. The Costs If We Do Not Meet Our Goals Patients will be harmed Provide VBP Report on hospital’s projected payment Provide data on HAC cost (You can obtain hospital data from PHA web site. Contact your Quality Director for instructions and password or contact Faizah Muheb at fmuheb@gha.org )

  12. As Leaders We Are Working To Create A Patients First Culture By Aligning Goals Aligning Behavior Aligning Processes We are stepping up efforts to hold everyone accountable and setting the clear expectation that everyone has a responsibility to hold each other accountable

As Patients First Leaders We Have Set Clear Objectives To • Create a culture to promote a patient/family safe care environment • Find insights/connections with existing efforts in learning activities • Set aside judging and critical mind sets; find ways to use new knowledge • Maintain persistent optimism with all staff • Embrace complexity; find joy in making things better for all • Share with others internally and externally as we have successes And above all, provide the WILL to execute, hold all ACCOUNTABLE and keep the drum beat going

Putting Patients First Will Require That We Find individuals to lead the Patients First movement in all areas of the hospital Actively welcome all staff as partners to be relentless putting patients first Demand that our patients and their families are included in all interactions regarding care Communicate regularly to all our improvement goals and results Make front line staff participation in solutions mandatory Identify, and support unit improvement champions

Leader Actions That Puts Patients First Provide support and resources for teams to identify and implement solutions that address improving care and increase efficiency Hold all in the organization, accountable for safe patient care Perform rounding to listen to staff and set expectations with managers, employees, physicians and patients Create a Just Culture where errors and near misses are freely reported and learned from Support an environment where everyone can speak up for Safe Patient Care

Leaders are Key to Success We already have successes to celebrate! • Our Vision includes (add some appropriate statements re patient and quality) • Expectations have been set through our participation with the Hospital Engagement Network • We are querying staff regarding additional support needed for the work • Time and resources to do the work • Rounding, asking staff about the improvement, knowledge, what is needed • Assessment of progress includes a review of measures in key meetings • Feedback mechanism keep staff aware of progress • We celebrate and spread success • Lead by example

Putting Patients First: Your Role With Leaders and Staff • Promote and hold us accountable for working together to promote: • Customer/patient focus • Unconditional team work • Boundarilessnessand bringing down silos • Decreased cycle time • Speed and agility of change • Iteration and evolving in real time • Net Forward Energy If you’re not working together, you’re not doing your job!

Take the Putting Patients First Pledge We pledge to: Make achieving the goals of Putting Patients First, harm reduction and improved care transitions to reduce readmissions priority of our Board of Directors, senior leaders, clinicians, and staff; Support and working each other in engaging patients and families in order to make care safer, improve communication, and increase coordination by implementing proven systems and processes; and Learn from and share with others our experiences with Putting Patients First, making care safer and more coordinated.
