WHY NEW PEP FORMS? • To incorporate the use of meaningful data as a pre-requisite to writing & reviewing PEPs • To support our move toward using the RTI “Response to Intervention” Model in UCPS • To improve the quality of teachers’ PEPs by aligning them with the NC SCOS goals and objectives • To integrate technology as a tool for writing PEPs that match RTI and use SCOS

WHAT’S NEW?CHANGES to the Elementary Level PEP FORMS in UCPS2008-2009

The on-line version is also grade-level specific.

UCPS PEP Form – NEW ITEMS When using the On-Line version, teachers will have a PULL-DOWN MENUlisting NC SCOS objectives at their grade level for each academic subject on the PEP form
UCPS PEP Form – NEW ITEMS In the On-Line form, teachers will use a TEXT BOX to type the specific skills, goals and strategies they plan to use to address the student’s areas of need
UCPS PEP Form – NEW ITEMS List the 2 most significant behaviors in observable terms that are interfering with student academic success. The Guidance Counselor, Psychologist, Curriculum Coordinator, or an Administrator must observe the student before a Behavior PEP is written. The new PEP allows for the development of a BEHAVIOR PEP!
UCPS PEP Form – NEW ITEMS List the 2 most significant behaviors in observable terms that are interfering with student academic success. The Guidance Counselor, Psychologist, Curriculum Coordinator, or an administrator must observe the student before a Behavior PEP is written. This new Behavior section of the PEP REQUIRES that a teacher create a Behavior Observation Log for taking notes about the student’s behavior over time as one form of data needed for this type of PEP.

The new Behavior section of the PEP also REQUIRES that teachers focus only on behaviors that interfere with learningAND that they enlist the help of a fellow professional to perform an additional observation before a Behavior PEP can be written.
UCPS PEP Form – NEW ITEMS The new PEP form asks teachers to identify the specific type of ongoing data they plan to use to monitor student success in meeting PEP goals.

(Print the PEP, get it signed and place it in the UPCS GREEN PEP folder).
IMPORTANT:MaintainingSECURITY & CONFIDENTIALITYWhen Using the On-Line Form
PEP SECURITY & FILE STORAGE for ON-LINE FORMS • BEFORE entering student information using the on-line form, security measures must be taken: • LOG IN as a teacher when using a school computer to enter PEP data & LOG OUT when finished to maintain security of PEPs OR • SAVE ALL DATA to a Flash Drive or Diskette that is secured at school when finished
GETTING STARTED USING the ON-LINE FORM: • Open the grade level PEP form needed and save it to the desktop first • Once it’s on the desktop, open the desktop form to create a new student file by clicking “Save As” and typing the student’s name • Fill out the form, remember to “Save” and then PRINT EACH FORM to place in green PEP folder • Repeat the process of opening the desktop form for EACH STUDENT & “Save As” (name) before writing it • When finished, save ALL of the student files in a new folder for PEPs and secure it
USING the ON-LINE FORM for MID-YEAR & END-of-YEAR PEP REVIEW: • OPEN each student’s saved PEP • Complete the form as before, using the appropriate line on the PEP (Moving to the 2nd line for a “Review” or the 3rd line for a “Final Review” found in each section of the PEP) • At the mid-year review or final review, print only the FIRST PAGE of the On-Line PEP form, as you will need to keep the original signature page the same for the entire year so that you may add new signatures to existing signatures. • At the end of the year, remove previous page 1 goals and attach ONLY the final copy of Page 1 of the On-Line form (which includes ALL goals written for the year) to the original signature page and place it in the GREEN folder used to collect PEPs in UCPS.
End of the YearChanges to the PEP At the end of the year, teachers will provide a list of recommendations for next year’s teacher based on their evidence of student success during PEP implementation this year.

The number of student absences must be recorded by each teacher and a vision and hearing screening must be completed for students recommended for Tier II intervention.
