THE EXPERT PATIENTS PROGRAMME Is an NHS based range of lay-led self-management programmes for people affected by long-term health conditions. They are shown to be effective in helping to both improve quality of life for individuals and positively aid health services as a whole
What does the programme involve? • Six weekly sessions each lasting two and a half hours. • Delivered by two tutors to a scripted manual licensed by Stanford University to the Welsh Assembly Government. • Courses are free of charge and run in the local community. • Delivered to mixed groups of up to 16 people • A handbook accompanies the course & is distributed free of charge.
The programme covers: • How to communicate more effectively with your healthcare team • Techniques to help you deal with difficult emotions such as anger, frustration & depression. • Taking care of your health – such as medicine, physical activity & healthy eating • Techniques to help you manage pain, fatigue & other symptoms • Ways to help you relax • Appropriate exercise for maintaining & improving strength, flexibility & endurance • Planning for the future
CORE SELF-MANAGEMENT SKILLS: • Problem solving • Decision making • Resource utilisation • Developing effective partnerships with health professionals • Taking action / Goal setting

Significant Patient Benefit Decreased • Pain / Fatigue • Anxiety /Depression • Sense of powerlessness • Unnecessary hospital admissions • Complications from their long term condition
