Background 1 Poorly performing EIA for chlamydia leads to introduction of new testing method, which also detects gonorrhoea Decision to run this new GC test (Gen-Probe Aptima Combo 2) alongside existing test (GC culture) for trial period Sensitivity of existing test ~ 95% Dual NAAT Testing : cost neutral AND THAT’S SOMETHING Men urethral microscopy and culture Females : cervical microscopy, and urethral and cervical culture Selective Antibiotic Medium : Vanc/Nystatin/Trimethoprim/Colistin Rectal swabs not performed unless anal sex. TMA Combo 2 assay : target 16S rRNA : Roche Assay This gene present in sub flava and lactobacilli .THIS HAS GIVEN B1 NAATS A BAD NAME Van Doornum et al 2001, Journal Clin Micro , females, population prevelance of 1.2%, PPV 35% for roche PCR when a second confirmatiory 16s rRNA assay used or the LcX assay used Same study 5 positives of 500 for LCX all cult postive . Assay now discontinued? Comercial not performance reasons 2nd study : 5 case reports of +ve roche pcr naat in hawwai (done private lab in low risk monognomous realtionships, mostly with BV), all 5 tma –ve and cult neg. Little if any published data on in vitro data on ability of these tests to detect non NG species. Aptima Combo 2 American Multicentre Study : female endocervical and urines GC Prevalence about 8% ‘Gold standard’ cult positive or LCx positive (two sites) 126/1479 : Combo 2 positive Sens 99.2% Spec 98.7 % PPV 90% : No data on PPV as culture ‘gold’ standard Gaydos et al J Clin Micro 2002 : FDA approval for detection using urine and cervical swabs. But of 17 false +ve naats in this study, 15 were confirmed by another TMA NAAT targeting another sequence one was TMA NEG BUT A TRUE postive , no data on whether it was a cult pos or an LCR post data on cross referencing , nice to see USA suffers from same problems as in UK on this issue. Note for the chlamydia part of study specificity was 97.4% and PPV 88%. Confirmatory tests roche PCR and LCx.Gaydos et al J Clin Micro 2002 : FDA approval for detection using urine and cervical swabs.
