Triple Aim for Populations Our Goal: Drive the Triple Aim, simultaneously improving the health of the population, enhancing the experience and outcomes of the patient, and reducing per capita cost of care for the benefit of communities.
Next steps…on the ground • Development of Wellbeing Hub, WHO Age Friendly Cities and Social Enterprise projects developed with Councils and other partners (making it real) • Core of the Making Life Better plan • Core of the 4 Community Plans • Priority for the Trust…not urgent but Important • An integrated approach enabling the Trust to deliver its statutory duty for Health and Wellbeing Ultimately leading to a better society with a HSC we can afford
