CCO’s goal: The right care, at the right time, in the right place • High-quality “family-centered” care • Involving OHP members in decisions about their care • Focusing on prevention • Making sure people get right medications and tests • Making sure people understand your follow-up care • After-hours help, alternatives to the emergency room • Better communication and coordination among health and behavioral health providers
