What is Person Directed Care? • It’s a philosophy that says the person is more important than the system and • the system should serve the needs of those within it.
Create systems within which individual choice is honored Promote a climate where all thrive, flourish, and grow Promote the abilities and optimal level of function for all people Support the integration of quality of life and quality of care by focusing on the Holistic Model. Promote quality improvement as a responsibility of all staff. Care Practices
Make the creation of meaningful and lasting relationships a priority (staff, residents, family) Provide opportunities for individuals to make empowered decisions and take greater responsibility Develop teams that will guide the organization to continuous improvement.
