3. Young Persons Support Service Service launches 1st July 2008 “Our vision is to provide a high quality, truly integrated homelessness support service accessible to all young people in Hertfordshire”

4. Young Persons Support Service Three service areas: • Advice, Information and Mediation • Crashpad • Floating Support

Young Persons Support Service Integrated Practice: • HYHG and Aldwyck are a uniquely placed partnership driving the development of integrated services for young people in Hertfordshire • Meet countywide objectives (Local Area Agreements (LAA)) • Continue membership of all relevant strategic groups within the county • Influencing county and district young persons strategies

Seamless Service: Single referral process via Gateway Advice and Information new focused service Floating Support used as prevention tool -starts with Service User at an earlier stage – at home or in Crashpad Mediation – new integrated approach Mediation skills used for early intervention Services flexible to meet demand New Integrated Service

8. Consistent standards and definitions of Floating Support across the County Partnership with Aldwyck facilitates access to supported accommodation Efficiencies through increased work with specialists agencies Flexible countywide service in response to levels of demand New Integrated Service

Putting Young People at the Centre • Service Users involved in all aspects of strategic and operational service delivery • Ease of access to initial Gateway through various means • Accurate single assessment process to inform referral and signposting • Service Users able to define and own their Support Plans • Service Users are integral to the monitoring, review and on-going development ofservice provision

Young Persons Support Service Main Changes Summary: • New “Gateway” access to all services • Advice & Information – new service for 16-25’s • Earlier intervention from Floating Support • More integrated approach to mediation • Service offer geographically flexible • Consistency of Floating Support service delivery • Crashpad will take care leavers from 16 – 21 • Crashpad growth by 50%
