Day 1 Day 30 Day 46 Day 90 • Disability begins • Benefit Waiting Period ends • Claim is approved • Employee begins receiving STD benefit • Maximum Benefit Period ends • Employee receives final STD benefit payment Maximum Benefit Period andBenefit Waiting Period In this example, the disabled employee has 45 days of sick leave, and the STD benefit becomes payable on Day 46. The employee receives a STD benefit for 45 days. The maximum number of days you may receive a STD benefit depends on the Benefit Waiting Period, the amount of sick leave, vacation time and PTO you have, and the plan you select. Policy: GP399-STD

Insured predisability earnings$800/week STDbenefitpercentage x 60% $480 Less deductible income -50 Amount of STD benefit$430/week Note: If you choose Option 1, your STD benefit will be $250 a week regardless of your income. Submitting a Claim Is Easy • Access The Standard’s Web site for Hennepin County employees (www.standard.com/mybenefits/hennepin) to obtain a claim form • If you don’t have access to the Web site, call the Human Resources Benefits office at 612.348.3530 to request forms • Submit your claim • The Standard will contact you shortly after receiving your completed claim form Policy: GP399-STD
