Overview of Healthy Start • Implemented on April 1, 1992 • Reduce infant mortality, reduce the number of low birth weight babies and improve health and developmental outcomes • Offers universal prenatal and infant risk screening to identify pregnant women and infants at risk for adverse birth, health and developmental outcomes. • Offers care coordination services that support families in reducing factors that place pregnant women and infants at risk for poor outcomes.

Introduction • Grant awarded in October 2002 to the Healthy Start Coalition of Miami-Dade in partnership with the Economic Opportunity Family Health Center, Teen Pregnancy Prevention Center, and Health Council of South Florida. • Funds are available from the United Way of Miami-Dade Success By 6 Program. • The goal of the Initiative is to increase the number of completed Healthy Start prenatal screens by conducting intensive outreach to pregnant women and teens in zip code 33147 (Liberty City).

6. Zip Codes of Participation • The highest percentage of women came from zip code 33147 • In the second six months of outreach prescreened women came from 24 additional zip codes (27 versus 51 zip codes of participation)

Achievements for Year 1 • 20.4% of women from 33147 were aware of Healthy Start; overall 19.7% were aware the program • In 33147, 1.9% were aware because of Initiative activities compared to 11.1% in 33142 and 4.2% of all other zip codes • There is an opportunity challenge to raise awareness of Healthy Start • 100% of prescreened women from 33147 were assigned to Healthy Start providers

14. Prenatal Screening Results for Year 1 • Based on the Self-Referral forms, the leading reasons for referral were: stress (33.3%), no medical coverage/insurance (9.3%), and unemployed/partner unemployed (7.4%)

Process Achievements/Limitations • Outreach partners each met their goals for contacts (405 contacts) • Direct outreach (i.e. street-level) was the most successful • Follow-up procedures were perceived as labor intensive and unrealistic • Each outreach partner adopted their own method of follow-up • Outreach partners found that prescreened women came to expect a high level of interaction with Healthy Start case managers

Recommendations • Efforts in year two should focus on: • Exploring new locations for outreach • Utilizing high visibility, public events as opportunities for collaboration among outreach partners • Building/strengthening relationships with medical providers • Implementing or developing new follow-up procedures
