Key Service Elements • Receive NHS Health Check from provider through lifestyleservices@nhs.net • Follow up with consenting client within 72 hours of receiving dataset • Attempt to contact client at least 3 times • Follow up with range of methods (phone, text, email, letter) • Follow up at a range of days, including Mondays – Saturdays, and times, including late evenings until 7pm, to ensure working clients can access support
