  4. WHO: Name/# of the squad, platoon, company, battalion being trained: 1st Sqd, 3rd Plt, Co B, 301 Bn

  5. WHAT: Name of the class or activity that is happening (i.e. Class: Military Courtesy or PT Test)

  6. TRAINER: Who is primarily responsible for this activity (by name and rank such as C/SSG Jones)

  7. UNIF: What uniform will cadets wear (Class A, B, C, PT, Civilian Clothes?)

  8. NOTES: What supplies will you need, what CACC curriculum materials will you use? (handout 13-H-5, basketballs)

  9. LOCATION: Where will training take place? (Classroom 214, Football field, etc)

  10. CACC Standard and Objective: Which standard and objective from the master list is being addressed in this training activity? It can be more than one.
