CHC Is A “Medical/Health Care Home” • “Patient centered” health care provider • Physician directed health care team • Regular source of continuous primary and preventive care • Primary and preventive care is integrated and coordinated • Provide comprehensive, quality, safe care • Provide enhanced access to care – through systems communication options Community Health Centers of Arkansas, Inc.

CHCA HIT Vision • Enhance the provision of quality patient care and patient safety • Continue to focus on the reduction and elimination of health disparities • Increase operational efficiency and cost effectiveness • Implement EMR and enhance PM systems at all 12 CHCs and their 69 locations Community Health Centers of Arkansas, Inc.

CHCA HIT Vision (Continued) • Participate in health information exchange: • Within each individual CHC system • Within the CHCA Healthcare Delivery System (All 12 CHCs) • With our partners • Increase workforce development through distance learning opportunities Community Health Centers of Arkansas, Inc.

  5. Current CHC HIT Status • All 12 CHCs are utilizing Practice Management (PM) systems • 8 CHCs have Electronic Medical Records (EMR) Systems • 4 CHCs implementing EMR Systems within the next year Community Health Centers of Arkansas, Inc.

AR Health Center Controlled Network (AHCCN) • Award: $458,003 - Project Period: 09/01/2009-08/31/2010 • Impact: Enables remaining 6 CHCs to implement EMRs and upgrade PMs • Includes Health Centers locations in 25 Arkansas Counties and approximately 61,000 patients • Network Members: • CABUN Rural Health Center, Inc. • Corning Area Healthcare, Inc. • East Arkansas Family Health Center, Inc. • Jefferson Comprehensive Care System, Inc. • Lee County Cooperative Clinic, Inc. • Mainline Health Systems, Inc. • Community Health Centers of Arkansas, Inc. Community Health Centers of Arkansas, Inc.

  7. AHCCN Phase I • All Members have signed EHR/PM Vendor Contracts • All Members have purchased EHR/PM Systems • All Members have identified IT Champions Community Health Centers of Arkansas, Inc.

AHCCN Phase II • All Members have finalized an EHR/PM implementation plan • All Members have resolved key issues related to security, HIPAA compliance, PHI, etc. • All Members have hardware and software in place • All Members have completed Provider Training Community Health Centers of Arkansas, Inc.

AHCCN Phase III • Establishment of a secure, clinical data sharing platform • Selection and establishment of a data warehouse to monitor and aggregate clinical data for system quality improvement • Establishment of data sharing interfaces with data warehouse • Reporting on multiple performance outcome measures with national benchmarks Community Health Centers of Arkansas, Inc.

CHCA HIT: AHCCN Goals • Share “Best Practices” in the EMR/PM implementation process • Enhance chronic disease management and obtain improved health outcomes while eliminating health disparities through EMR/PM implementation • Reporting Measures: • Diabetes-HbA1c % • Cervical cancer-PapSmear Screenings • Preventive medicine-Childhood Immunizations • Cardiovascular diseases-ControlledBloodPressure for HTN patients • Focus on Risk Prevention and Management Community Health Centers of Arkansas, Inc.

AHCCN Major Accomplishments • Participated in Volume Purchase Savings for Computers, Scanners, and Servers • Mainline Health Systems implemented its EHR/PM System on March 1, 2010. • Selected VirtualCHC (Michigan PCA) as Network EHR/PM hosting and/or backup vendor on March 31, 2010. • Corning Area Healthcare implemented its EHR/PM System on April 26, 2010. • Extended Membership Invitation to all Arkansas CHCs on May 4, 2010 Community Health Centers of Arkansas, Inc.

A Project In Progress… • AHCCN Members will: • Implement EHR/PM systems by March 2011. • Establish a clinical data sharing platform to aggregate/analyze quality measures across multiple organizations and locations. • VirtualCHC will prepare AHCCN Members for: • Meeting “Meaningful Use” requirements for incentive payments • HIT System Security/Redundancy • Health Information Exchange (HIE) Community Health Centers of Arkansas, Inc.
