  2. Fair Housing, Olmstead and Money Follows the Person: Increasing and Leveraging Affordable, Accessible Housing for Persons with Disabilities

Money Follows the Person The Money Follows the Person (MFP) Program, currently funded in 29 states and the District of Columbia, is designed to: Transition people who have lived in nursing facilities or other qualified institutions to community-based housing, and Change State policies so that Medicaid funds for long-term care services “follow the person” to the setting of his or her choice.

  4. Qualified participants are defined by the following: Reside in an institution (90 days minimum). Institutions include: hospital, nursing facility, or ICF/MR. An IMD is also included to the extent that medical assistance is available under the State plan for service in the IMD. Receive Medicaid benefits for inpatient services furnished by such inpatient facility. Need HCBS services in order to successfully reside in community based settings. Who can be served?

  5. Qualified HCB Program Services Enhanced FMAP Must be continued at the conclusion of the demonstration via waivers or State Plan. HCB Demonstration Services Enhanced FMAP No requirement to continue beyond the demonstration. Supplemental Demonstration Services Regular FMAP Cannot receive Federal reimbursement beyond the demonstration. What services are received?

The MFP program defines “qualified residences” to include the following: A home owned or leased by the individual or the individual’s family member; An apartment with an individual lease, with lockable access and egress, and which includes living, sleeping, bathing, and cooking areas over which the individual or the individual’s family has domain and control; and A residence, in a community based residential setting, in which no more than 4 unrelated individuals reside. Where do transitioned individuals live?

  7. The MFP states originally projected to transition 35,380 individuals. As of 2010, and after several years of experience, the MFP states have set reduced interim goals, for a national total of 23,352 individuals. How many people will transition?

  8. Implementation has been more difficult than expected. The national evaluation of the program identified three themes that affect implementation: Problems/delays meeting planning and data reporting requirements; Adverse impact of the economic downturn, and; Implementation Issues

  9. Community-level barriers such as lack of affordable and accessible housing and rental assistance vouchers. In an effort to address this problem, CMS has funded a new initiative entitled: “Housing Collaborative under the MFP Rebalancing Demonstration”. Implementation Issues

  10. The new Housing Collaborative initiative is designed to develop, strengthen and support strong and sustainable housing and service system collaborations at the Federal, State and local levels. Housing Collaborative Initiative

At the federal level, building on the work of the Community Living Partnership, this initiative will: Assess the knowledge, willingness and capacity of key staff at HUD, CMS and other HHS agencies regarding the housing needs of persons with disabilities. Offer cross-training and team-building opportunities, linked to meaningful action, that will help participants move from parallel action to collaborative, problem-solving partners.   12. At the State level, this project will identify five key states and will work with those states to increase the availability and accessibility of affordable housing. Cross-agency collaboration, combined with the creative use of resources, is a central strategy for all selected pilots. Housing Collaborative Initiative

  13. At the local level, this initiative will identify up to 15 housing pilot projects. These projects will be focused on increasing the number of housing units, in combination with an array of services and supports designed to promote housing retention. Housing Collaborative Initiative

  14. The Housing Collaborative initiative will help to move the MFP agenda forward, through identification and elimination of barriers to meaningful interagency collaboration. Housing Collaborative Initiative
