IMPORTANT! • “Each local school board shall secure the approval of the Director of the Public School Facilities Authority or his designee prior to the construction or letting of contracts for construction of any school building or related school structure, or before reopening an existing structure which was formerly used as a school building but which has not been used during the previous year.”
NEW MEXICO STATUTE • The New Mexico Statute requires all schools and/or districts planning to engage in construction projects to seek the approval of the PSFA prior to construction or the letting of contracts (section 22-20-3, NMSA,1978). • Definition of Construction (section 13-1-40, NMSA 1984).
FILLING OUT THE FORM • There are two versions posted on the PSFA website. • The “hardcopy” version can be printed and filled in by hand for submittal. • The “electronic” version is intended to be filled out electronically prior to printing for signatures and submittal.

Name of school(s) AFFECTED • Name all schools which will be affected by the proposal. • Also include each school’s designation as elementary, intermediate, middle school, etc.
BRIEF DESCRIPTION OF PROPOSED PROJECT • Provide a short description of the proposed project. • If multiple schools, provide a short description for each. • Provide the exact award language (if applicable).
CURRENT ENROLLMENT(S) • Typically this is the number of students enrolled at the affected school(s) or projected student population of any new facilities.
GRADE LEVELS • Please include the full grade range: • For example: if the project will affect both a middle school (6-8) and a high school (9-12), enter 6-12.
DESIGN CAPACITY: • This refers to the maximum number of students the facility is designed to support. • Include figures for maximum number of students served by permanent buildings only, including the design capacity of the proposed project. • Do not include figures for portable classrooms in the design capacity.
DISTRICT REPRESENTATIVE: • This shall be a DISTRICT EMPLOYEE who has the authority to represent the district on the project. This may be the district’s superintendent, a member of the district administration staff or another district employee. • Providing this information can help expedite the approval process.
ARCHITECT • Provide the name and contact information of the architect or representative of the architectural firm. • This will help expedite the approval process.
REVIEW PHASE: • The PSFA requires that documentation for any construction project be submitted at four separate phases of development. • At each phase in the development of a project, a new form must be submitted along with the required supporting documentation and all required signatures. • Often the only change is checking the appropriate box to designate which phase.
On a separate sheet (8 1/2 X 11) provide a statement describing how this project serves the needs of the school(s) and/or district including a statement of need and the educational programs to be served. On a separate sheet (8 1/2 X 11) include a map indicating the following within a five mile radius: Proposed project Existing attendance centers Obstructions PROGRAM STATEMENT
SCHEMATIC PHASE (30%) • Submit with 2 sets of drawings and include: • site plan • a brief description of proposed mech. and electrical systems • a description of any special or unique conditions
DESIGN DEVELOPMENT PHASE (60%) • Submit 2 sets of drawings that provide a fair representation of the scope of project to include: • civil • structural • architectural • mechanical • plumbing • and electrical drawings
BID DOCUMENTS (100%) • Submit with two sets of complete, full-size, construction drawings; • and two complete sets of of specifications.
ESTIMATED COST & SOURCES OF FUNDING • Provide information for all funding sources tied to the specific project. • For Legislative Appropriations be sure to include the Bill # and the year enacted. • For other types of funding not listed on form, fill in the amount as Other and identify the source in the space provided.
REQUIRED SIGNATURES • All forms received without all three required signatures will be considered incomplete and will be returned to the district to be completed for resubmission. • Required Signatures: • Architect • District Representative • PSFA Construction Manager
