Dynamic to ensure communication necessary for transition. TAP: Dynamic and changing to facilitate transitional planning TAP Evolves: TAP Evolves: TAP Development of Case Management Plan Institutional Programming Creation of the Parole Supervision Plan Release from Prison Creation of the ReEntry Plan • about 90% of all releases are to supervision Transition Planning Work with offenders while they are incarcerated preparing them for release and continue to work as partners as needed to ensure community success. Transition Teams Include transitional planners, parole reps, service providers, offender, and his/her family
