  1. University of Chicago Medical Center Comer ER Transfer Process 1 2 The following conditions will automatically prompt transfer. CHRONIC CONDITIONS: 1. Sickle Cell 2. Diabetes 3. Asthma Q2 *That do not require PICU, Step Down or sub-specialty consultation or imaging other than endocrinology or hematology. TRANSFER STEPS: 1. UCMC ED completes EMTALA & transfer information. 2. UCMC ED RN calls RH unit for RN to RN report and report faxed after call. 3. UCMC sends copies of medical records, labs, images, & EKG to RH. 4. UCMC arranges BLS ambulance transfer to assigned RH nursing unit. 5. RH Bed Control will maintain log of patient for tracking. 6. RH will develop discharge plan. 7. Records of first 10 transfers will be reviewed by Program Overview team to evaluate process. *Receiving Hospital (RH)
