Intro to Person Centered Planning Person Centered Planning underlies and guides respectful listening which leads to actions, resulting in people who: • Have positive control over the life they desire and find satisfying, • Are recognized and valued for their contributions to their community • Are supported in a web of relationships, both natural and paid, within their communities

PCP and Self-Determination • Person Centered Planning is a step with the Self-Determination process. • The service recipient is the center and the driver of the process. • Self-Determination is a special funding program of the regional centers where clients have greater control over their budgets and spending plans.

  15. PCP Models Essential Life Style Plans • Focuses on what is important to someone now and balancing this with things that we need to know or do to keep the person healthy, happy and safe. • Action Plan • Living document that changes and grows with the person. Maps • Uses a combination of process and graphics to create a shared vision • Focuses on gathering information for planning based on the story of a person. • Places the individual at the center of the process Paths • Moves people through a defined set of questions • Start with the future and work backwards

  16. Paul PCP Model • Paul had his PCP meeting on July 3rd • Paul Invited people who knew him well and cared for him • Paul used the Map method to guide the meeting • The meeting covered Paul’s strengths, nightmares, supports, dreams, and first steps.

  18. Take-Aways from the Process • We often go through life with out seeing our strengths and accomplishments • Participating in the PCP process helps you see the positive qualities and accomplishments you bring to life.

Future of Self-determination And Self-Advocates Importance to Them • In two years, Self-Determination will open up to all clients who want it. • Self-Determination will radically change the way services will be delivered in California. • Participating in the Self-Determination program will empower you to be a more active and vocal self-advocate.
