Step 13: Personalized Solutions Assessment • Creating PSA • Important to know the agencies you are referring to • Keep in contact with outsourced referrals to know they are current • Spend time getting feedback about referral agencies from patients
Step 13: Personalized Solutions Assessment • Completing PSA • Advocate will typically complete PSA during Step 11, while nurse reviews HQ with patient • Additional referrals/questions should be managed through a referral resource directory, or directed to PR Director • Scheduling Return Visit • Managed by Advocate, but must be discussed with Nurse/Sonographer to determine need/timeframe • Give patient first available on same team schedule • May need to change when reviewing PSA with patient • Copy of PSA is kept in chart
Step 13: Personalized Solutions Assessment • Delivering PSA • Advocate will review PSA with patient immediately following Ultrasound • Discuss Return Visit with patient • Church Referral is the introduction to Gospel Presentation
Step 13: Gospel Presentation • OT P. 54 • Ask if patient is currently involved in a church, and show her church referral on PSA • Gospel presentation is permission-based, using script: • “Can I share with you a little about what the bible says about going to Heaven and having a relationship with God?” • Patient may say “no,” and Advocate will continue on to Step 14.
Step 14: Exit Process • OT P. 55 • Ask 2 closing questions • Ask her to complete the Exit Survey • Just like the WCYE, the Exit Survey does not go into the patient chart • It is used for demographic research/marketing decisions • Exit Survey is returned to the PR Director
