  1. National Drilling Company DROPPED OBJECTS HUNT DAY By MORCHED ALILA Senior Safety Engineer November 29, 2010

  2. DROPPED OBJECTS HAZARDS

  3. NDC EXPERIENCE WITH DROPPED OBJECTS HAZARDS • “HIGH-POTENTIAL NEARMISSES/INCIDENTS” • IN 2009 NDC recorded some HIPO NM’s related to Dropped objects including: • Dropped tools while working at height. • Dropped fill up nose while running casing (45’ to rig floor). • Dropped drill line clamp (90’ to rig floor) • Additionally , rigs reported many dropped objects hazards during weekly inspection carried out on the derrick that are immediately rectified. • IN 2010 NDC recorded some HIPO NM’s related to Dropped objects including: • 4 dies dropped from topdrive grabs 90’ to rig floor. • Heavy gear dropped 1ft while being dismantled by jacking crew • Half moon grating dropped to lower wellhead tower deck after being snagged by other grating.

  4. NDC DROPS HIPOs 2009

  5. NDC DROPS HIPOs 2010 Dropped heavy gear pinion Dropped Grating

  6. NDC DROPS HIPOs 2010 Pipe Handler Grabs Dies Dropped 90’ to rig floor

  7. IT’S STILL HAPPENING – REAL HAZARDS “So what are we doing about it?”

  8. NDC DROPS CAMPAIGN (MAY-AUG 2009 ) • LAUNCHING CAMPAIGN • Determined to eliminate dropped objects, we launched the Prevention of • Dropped Objects campaign in May 2009. • All rigs have participated in this campaign (18 Onshore + 10 offshore rigs). • Campaign material was purchased from UK and distributed to all rigs . • Posters • CAMPAIGN OBJECTIVES. • Raising awareness of personnel about the hazards in order to improve their perception of hidden risks related to dropped objects hazards.

  9. DROPPED OBJECTS HAZARDS CAMPAIGN (MAY to AUGUST 2009) • Distribution of professional videos related to DROPs hazard that are used world wide • New Driller log book for DROPs prevention where all tools or equipment to be used at height on the derrick should be logged and cleared at the end of the job

  10. RAISING AWARENESS OF PERSONNEL • Provide In-house trainings to explain different cases of DROPs hazards using power point presentations developed by HSED and presented by FSE’s during safety meetings

  11. RAISING AWARENESS OF PERSONNEL

  12. RAISING AWARENESS OF PERSONNEL

  13. RAISING AWARENESS OF PERSONNEL • Score for all rigs for 8 months (May to Dec. 2009) has proved good performance enhancement

  14. NDC DROPs HUNT DAY / FEEDBACK DECEMBER 9, 2009

  15. RAISING AWARENESS OF PERSONNEL • Announcement of the NDC DROPs hazard Hunt day after management and clients approval and full support of the action and provision of necessary guidelines. 9th December 2009

  16. DROPs HAZARDS HUNT DAY ANALYSIS • The action covered all Onshore and offshore rigs. • The Findings • Drops hazards related to the derricks ( 100) • Non-derrick drops (88) • Loose tools and items (64) • Unsecured equipments (90) • Openings, toe boards (40)

  17. Action item No.3 Broken beam clamp New beam clamp DROPs HAZARDS HUNT DAY FINDINGS • The most HIPO drops– ND-38 broken mounting beam clamp for fluorescent light

  18. DROPs HAZARDS HUNT DAY FINDINGS • ND-38 A-Frame pin without safety bolt. ND-31 Torque Tube Tee Bar No.2 ( 1 Nut Loose) ND-32 One finger on monkey board without safety chain To be welded on next rig move.

  19. DROPs HAZARDS HUNT DAY FINDINGS ND-24 cable tray pin short & w/o safety pin ND-34 Missing self closing door on monkey board and Unused snatch block was hanging on handrail for monkey board and Opening on floor for crown block

  20. DROPs HAZARDS HUNT DAY ANALYSIS • Conclusion and Summary: • Inspection is more efficient if carried out by a team rather than by an individual • Dropped object campaign and training enhanced the quality of hazard identification and reporting • Dropped Objects hazards will continue to exist on the rigs and continuous vigilance and good risk perception is a key to reduce number of related incidents
