For example: • Patient-Centered Medical Homes • Shared Decision Making • Regular assessment, goal setting and problem solving support • Skills enhancement • Referral to community resources • Interactive educational interventions that build confidence and skills such as goal setting, decision making, problem solving, and self-monitoring such as: • Chronic Disease Self Management Education • Diabetes Self Management • Mental Health and Depression Self-Management • Falls Prevention, Matter of Balance, Tai Chi • Activity programs like Enhance Fitness • Nutrition programs like “Healthy Eating” Prepared, proactive Practice Team Informed activated Patient Community • Knowledge-based instruction by • Health providers on a wide variety of • issues including: • Wellness, prevention, health promotion • Nutrition education • Tobacco cessation • Surgical patient education for a better recovery\ • Disease specific education • Use of mobile and wireless devices • to help individuals monitor their health, interact with providers, track and change their behavior and effectively self-manage.
