  2. 3rd Why / Failure Mode Development D4 - Developing the 3rd Why / Failure Mode …Options The 3rd Why defines the Failure Mode of the 2ndWhy/Requirements Option 1:Use the Process Sequence Example ‘VALVE MANUFACTURING’ Withadditional Pop-Up aid RAW MATERIAL DESIGN/ APPLICATION MACHINING TAPPING ASSEMBLY/ TEST Enter a Failure Mode directly into the cell/select from the drop down list if previously entered. Failure Modes are automatically added to the database. To Update/Edit/View the Database: Select ‘Add-Ins’, ‘Database Rankings’ , ‘Potential Failure Mode DBS’.

  3. 3rd Why / Failure Mode Development D4 - Developing the 3rdWhy / Failure Mode …Options The 3rd Why defines the Failure Mode of the 2nd Why/Requirements Option 1:Use the Process Sequence Example ‘VALVE MANUFACTURING’ Option 2: Use a Circumstantial approach Example ‘TEAM DISAGREEMENT’ NERVES ASSOCIATED ORGANS POSTURE SPINAL COLUMN MUSCLES DESIGN/ APPLICATION ASSEMBLY/ TEST RAW MATERIAL MACHINING TAPPING INCONSIDERATION COMMUNICATION DEADLINES FRUSTRATION PERSONALITY Option 3: Use any other logical method or sequence, Example ‘BACK ACHE’

  4. 3rd Why / Failure Mode Examples D4 - Developing the 3rd Why / Failure Mode …Options The 3rd Why defines the Failure Mode of the 2ndWhy/Requirements Option 2: Use a Circumstantial approach Example ‘TEAM DISAGREEMENT’ COMMUNICATION INCONSIDERATION COMMUNICATION DEADLINES FRUSTRATION PERSONALITY Enter a Failure Mode directly into the cell/select from the drop down list if previously entered. Failure Modes are automatically added to the database. To Update/Edit/View the Database: Select ‘Add-Ins’, ‘Database Rankings’ , ‘Potential Failure Mode DBS’.

  5. 3rd Why / Failure Mode Examples D4 - Developing the 3rd Why / Failure Mode …Options The 3rd Why defines the Failure Mode of the 2ndWhy/Requirements Option 3: Use any other Logical Method or sequence Example ‘BACK ACHE’ ASSOCIATED ORGANS SPINAL COLUMN POSTURE NERVES MUSCLES ASSOCIATED ORGANS Enter a Failure Mode directly into the cell/select from the drop down list if previously entered. Failure Modes are automatically added to the database. To Update/Edit/View the Database: Select ‘Add-Ins’, ‘Database Rankings’ , ‘Potential Failure Mode DBS’.

  6. 3rd Why / Failure Effect Development D4 – Two Failure Effects can be entered, First Failure Effect shows in the selection, Second Failure Effect only gets reflected on FMEA. PLEASE NOTE: The First Failure Effect should always be the one with the Highest risk (severity). Enter the severity directly into the cell or select from the possibilities in the drop down options. Both Failure Effects are pulled through to FMEA Withadditional Pop-Up aid

  7. 3rdWhy / Failure Mode-Effect FMEA D4 – 3rd Why = the Failure Mode and Effect Will automaticallybe updated 3rd Why / Failure Mode • Select which Failure Mode you would like toFurther Investigate RAW MATERIAL MACHINING TAPPING ASSEMBLY/ TEST DESIGN/ APPLICATION CUTTING FLUID VALVE BODY ADAPTOR SETTINGS SWARF FOREIGN MATTER VISCOSITY 4th Why 4th Why 5th Why

  8. 3rdWhy / Failure Mode C&E (Fishbone) D4 - The 3rd Why will be forwarded to the 3rd Arm/s of the Cause & Effect Diagram RAW MATERIAL DESIGN/ APPLICATION MACHINING TAPPING ASSEMBLY/ TEST

  9. This completes the D4 3rd Why Now start the D4 4th Why
