About Senior Resource Connection Since our beginning in 1956, the mission of the Senior Resource Connection has been to collaborate with community resources and provide services that support seniors, adults with special needs, and their caregivers to maintain quality independent lives. Over the past 60 plus years the organization has grown from a one room storefront serving Montgomery County to a multi-purpose community-based service agency that serves not only Montgomery County but the Greater Miami Valley and beyond. The organization strives to be in the forefront of developing new program initiatives and providing critical services for our community’s older adults and special populations. On an annual basis the organization serves well over 10,000 individuals and their care givers. The Senior Resource Connection is a 501c3 private non-profit organization.
Meals on Wheels • Meals on Wheels, the Senior Resource Connection’s Nutrition Program delivers hot, nutritionally balanced meals on a daily basis to eligible elderly and disabled individuals in 8 Counties in the Miami Valley area. • More than a meal, our trained drivers provide a friendly and daily personal contact that improves our customer’s and their family’s sense of well being. It is definitely reassuring to know that someone will check on a loved one daily, when you are unable.
Meals on Wheels • Who can get Meals-On-Wheels? • Generally a person is 60 years and older and is unable to prepare their own meals on a regular basis, due to being physically or neurologically impaired and has no one available to prepare meals. • The program also addresses the nutritional needs of individuals under 60 who are afflicted with some type of disability.
Meals on Wheels • What are the meals like? • Hot nutritious and delicious meals are delivered to your door. • All Senior Resource Connection meals provide at least one third of your daily required nutrients and are delivered with milk and bread. • Special diet/therapeutic meals are available and include low salt, low sugar, low fat, mechanical, therapeutic and renal. • A kosher meal is also available.
Meals on Wheels • Who pays for the Meals-On-Wheels? • Meals-On-Wheels is funded from a variety of sources some of which include: the Area Agency on Aging Title III, United Way, and is a passport provider in all counties noted. • The program also works with levy funding programs in all counties in their service area. • The Meals-on-wheels program accepts contribution’s and fee for service. • Program staff is very familiar with all available funding sources.
Meals on Wheels • How to get started: • Call the Senior Resource Connection at 937-228-3663 (food) or 1-855-854-8778. • We will work with you to determine you or your loved ones program eligibility. • We will then start delivering a hot meal between 10:30 AM and 2:00 PM five days a week. • Frozen meals can be delivered for evening/ weekends as pre-arranged.
Visiting Home Care • For non-medical help and assistance there is nothing better than our Visiting Home Care services. • Whether you or a loved one needs assistance with bathing, dressing or meal preparation or just need some extra help in light housekeeping, we can help seniors remain at home when health problems arise. • All of our home health care workers have successfully completed approved training programs. • All staff has completed authorized criminal background checks. VHC has staff on call 24 hours a day in order to address the ongoing needs of our clients. • The VHC program is both PASSPORT and Comcare certified in Montgomery Co.

Visiting Home Care • How to get started: • Call the Senior Resource Connection at 937-223-8246 ext. 243 or 242. • A Registered Nurse will be scheduled to visit you and your family to develop a written individualized Plan of Care. • Based upon completing a comprehensive nursing assessment a professionally trained home health care worker will be assigned to you in order to assist in meeting your identified personal care needs. • Upon initiation of services, Registered Nurses visit at your home on a regular basis to evaluate staff performance and reassess your personal care needs.
Philips Lifeline™ • Lifeline is an easy-to-use medical alert service designed to reduce the risk of living alone. • In the event of a fall or emergency, help is available at the push of a button.
Philips Lifeline™ • The Lifeline Personal Help Button connects you to a trained Personal Response Associate (range may vary due to differing environment factors) who can send help quickly - 24 hours a day, 7 days a week.

Philips Lifeline™ • Here’s how it works: • When the Personal Help Button is activated, you are connected directly to a highly trained Personal Response Associate who will assess your situation. • A determination will be made whether you require a visit from a neighbor or a family member. • If you need emergency services, they can dispatch assistance, retrieve relevant medical information from your profile, and even let emergency personnel know if you have a pet, all while you're on the line. • There is no equipment to buy and no contract to sign.
Philips Lifeline™ • How to get started: • The Senior Resource Connection Lifeline can help you maintain your independence, and also give your family peace of mind. • For more information, please call 223-8246 x213. • They will walk you through the options and schedule a visit to install the necessary equipment.
Philips Lifeline™ • Testimonial: • Last October my grandfather fell for the second time in the bath tub and luckily this time he had the Philips Lifeline detect his fall. Within seconds the operator contacted my grandfather asking him if he needed assistance and within just few minutes I was contacted along with other family members. If it wasn’t for Senior Resource Connection and their helpful staff when purchasing the Philip Lifeline we would have no other choice but to have my grandfather live in the nursing home. Community Geriatric Assessment Nurse Program • This service provides Montgomery Co. residents age 60 and older an opportunity for a comprehensive health care assessment and referrals to pertinent other healthcare professionals. • Face to face assessments are completed by experienced Registered Nurses from the Visiting Home Care staff. • Fees for this service are provided by funding from the Montgomery County Human Service Levy. • For more information, please call 223-8246 ext. 241.
Case Management Services • This service allows for the in-home intervention by professional case managers, who assist individuals age 60 years and older in establishing a network of services that promote continued independent living. • The program is funded by the Area Agency on Aging PSA2 under Title III of the Older Americans Act. The service is for residents of Montgomery County.
Representative Payee Program • This service provides Montgomery County residents age 60 and older in need of an independent party to address the handling of their ongoing financial affairs. • The program is certified by the Social Security Administration as an authorized representative payee program. • All staff has completed authorized criminal background checks. • All client monies are maintained in protected accounts at a local FDIC Banking Institution and the program is insured. • The payee program works in conjunction with The Adult Protective Service Program and is funded by Title XX. • There are no fees to the client associated with this program.
Congregate Dining • This service offered in Montgomery, Miami & Darke Counties affords individuals age 60 and older the opportunity to have a hot nutritional lunch Monday thru Friday while enjoying the company of their peers. • All counties have multiple locations. • There is a suggested donation. • For more information regarding the program and locations, please call (937) 228-Food (3663) or 1-888-580-3663. • The program is funded by the Area Agency on Aging PSA2.
Senior’s Best Friend • Senior’s Best Friend is just one of the special services provided by Senior Resource Connection to homebound seniors and disabled people in local communities. A supplemental pet food program for our seniors who are currently receiving Meals-on-Wheels services. • Senior’s Best Friend prevents clients with limited resources from having to choose between food for themselves or food for their cherished pets. It enables individuals to keep their companion animals, providing a special quality to their lives and saving their pets from an uncertain fate.
