  3. Title Page • Student’s name, section • Complete Residence Address • Name of Parish • Parish Address, Tel no, Fax no. • Name of Parish Priest • Name of Coordinator and contact #s • Name of organization/ministry or nature of your involvement

  4. I. Time Card • Attach the time card • Get the total number of hours. • Ask your immediate coordinator to sign. • You do not need to forge if you were not able to bring your index card. • We do not want this project to make students cheat. It is your humble service to God’s people that is important 

  5. II. Reflection • Give a short description of your involvement this grading period. • What obstacles did you encounter and how did you overcome each? • Cite two values which you acquired through your involvement. Cite instances to support your answer.

  6. 4. Choose one characteristic of the Early Church (communal life, teachings, breaking of the bread, prayer : See page 48 of your book). Explain how you practiced it in your involvement.
