Within the last five years, AstraZeneca has provided over $3 billion in savings to more than 1 million patients throughout the US and Puerto Rico. 

    3. AstraZeneca Patient Assistance Programs AZ&Me™ Prescription Savings program for people without insurance AZ&Me™ Prescription Savings for people with Medicare Part D

    4. Must: Be enrolled in Medicare Part D Be prescribed an AstraZeneca medication available in the program Have an income of less than $30,000 if an individual or less than $40,000 if a couple Have already spent at least 3% of their annual household income this year out-of-pocket on their prescription medicines

ACCOLATE® (zafirlukast) ARIMIDEX® (anastrozole) ATACAND® (candesartan cilexetil) ATACAND HCT® (candesartan cilexetil-hydrochlorothiazide) CASODEX® (bicalutamide) CRESTOR® (rosuvastatin calcium) NEXIUM® (esomeprazole magnesium) PLENDIL® (felodipine) PRILOSEC® (omeprazole) PULMICORT FLEXHALER™ (budesonide inhalation powder) RHINOCORT AQUA® (budesonide) SEROQUEL® (quetiapine fumarate) SEROQUEL XR® (quetiapine fumarate) SYMBICORT ® (budesonide/formoterol fumarate dihydrate) TOPROL-XL® (metoprolol succinate) ZOMIG® (zolmitriptan) ZOMIG-ZMT ® (zolmitriptan)

Program Benefits Savings are based on income, and patients pay no more than $25 for a typical 30-day AstraZeneca prescription Program offers additional savings for 60-day and 90-day prescriptions Enrollment can be done by phone or on-line Prescriptions can be filled at the local retail pharmacy Enrollment is for the calendar year then reapplication is required
