Complete your Wellness Requirements: Earn additional HRA dollars, pay lower monthly premiums You are eligible to earn additional HRA dollars each Plan year upon completion of all three of your Wellness Requirements. Additionally, those that complete their Wellness Requirements will not be subject to the Wellness Program Non-Participation Fee ($75 monthly for Employee and & $75 monthly for covered spouse/domestic partner) that will be imposed beginning April 1, 2013. • How do I complete my Wellness Requirements? • Go to www.myFirstChoice.fchn.com • You will see a green box titled “Wellness Requirements”. • Details on the three Wellness Requirements are listed under this header. • Those effective on the medical plan January 1, 2013 will have until February 28, 2013 to complete your Wellness Requirements. • What are the Wellness Requirements? • Wellness Requirement 1:Complete your Biometric Screening Labs • Wellness Requirement 2: Meet with a Kadlec Health Coach • Wellness Requirement 3: Complete online Health Risk Assessment at www.myFirstChoice.fchn.com • The health information you provide is completely confidential. Your personal results are not shared with Kadlec Management, used to set premium rates, or distributed to any third party without your express permission. • All eligible participants (employee, spouse/domestic partner, dependents) on the Plan are allowed to participate in the three Wellness Requirements. Employee and covered spouse/domestic partners must both complete the three Wellness Requirements in order to receive additional HRA dollars and pay lower monthly medical premiums. • Those who enroll in the plan after January the remainder of the calendar year from their medical plan effective date to complete their three Wellness Requirements. Additional Dollars deposited in your HRA will not be used to reprocess prior claims. $75 Wellness Program Non-Participation Fee -$50 Fee is reduced by $50 if ALL 3 wellness requirements are completed, regardless of results -$25 Fee is reduced by $25 if member is a non-tobacco user. Standard Premium applies if will requirements are completed and a non-tobacco user. -$10 – Goal Achievement Discount applies if all three Wellness Requirements are completed, a non-tobacco user Please Note: Newly hired employees with no opportunity to participate in the annual biometric screen will not face additional premium charges until after the following group screening.
