INCIDENT : Water truck was heading north on a snow packed single lane road through a blind corner, when an approaching southbound pickup started sliding, striking the water truck on the driver’s side. • 2. ACTIONS : • Revised Driving Policy to include a Defensive Driving course • Installation of VDR technology in all vehicles. • Installation of flag on water truck • Reviewed expectations of chain usage on water trucks • 3. LEARNINGS: • The icy condition of the road was underestimated and the drivers of both vehicles were driving too fast for conditions. • If the vehicles had been chained up or in four-wheel drive (pick-up) the incident could have been prevented. • The use of overhead vehicle flags could have alerted the other driver that that there was a vehicle approaching. • IN YOUR OPERATION: • Are chain requirements clear?
