  1. Practice-Based Learning and Improvement (PBLI)ASTHMA MANAGEMENT PROJECT

In preparation for this Asthma PBLI project review: • 2007 NIH Asthma Guidelines • Pediatrics in Review September 2009; Practical Mgmt of Asthma; Wood and Hill • Pediatrics in Review October 2009; Asthma Epidemiology, Pathophysiology, and Initial Evaluation; Hill and Wood • Apply these guidelines during this project.

  3. Review 20 of your Continuity Clinic Patients charts who carry a diagnosis of Asthma or other equivalent term. • If you cannot identify 20 pts, then use other patients in your clinic practice for the purpose of this PBLI exercise.

  4. See the Handout for instructions • Complete the table. • Do not use patient names or other identifying information • Calculate the following: • Percentage of patients falling under each classification (or not classified) • Percentage appropriately classified, misclassified or not classified • Percentage of patients appropriately classified, are receiving medications in concordance with the NIH 2007 Asthma Guidelines? • Percentage of patients given a Written Asthma Action Plan • Percentage of patients given a recommendation for a follow-up appointment

  5. Make at least one change in your practice that would improve some aspect of your delivery of care to patients with asthma. • Review your findings and your plan for practice based improvement with your Continuity Clinic preceptor and Dr. Auerbach • After four to six months, repeat a chart review of another 20 patients. • Did your recommended practice changes have the desired impact on your delivery of care to patients with asthma? • Repeat this cycle every 4-6 months over a two year period.
