Consultation on 3 Locality Model. • Conwy • 3 Integrated Health and Social Care Centres where Community Health and Social Care staff work jointly. 2 more in development. • Gwynedd • Three locality teams with identified clusters and some operational Multi-Disciplinary meetings

8. Gwasanaethau Gofal Canolradd/ Intermediate Care Services • The Wrexham Intermediate Care Service provides short-term intensive multi-disciplinary Health and Social Care services in order to support early safe discharge from hospital, or to overcome the need for hospital admission in the first place. Support is also provided to enable informal Carers to continue in their caring role during the period of crisis and beyond. • Mae Gwasanaeth Gofal Canolradd Wrecsam yn darparu gwasanaethau Iechyd a Gofal Cymdeithasol aml-ddisgyblaethol tymor byr dwys er mwyn cefnogi cleifion i ddod adref o’r ysbyty yn fuan a diogel, neu’n golygu na fydd yn rhaid mynd i’r ysbyty yn y lle cyntaf. Rhoddir cefnogaeth hefyd i alluogi Gofalwyr anffurfiol barhau i ofalu yn ystod cyfnod yr argyfwng a thu hwnt.
